I. Introduction The prevention and therapy of osteoporotic disorders in men is virtually unexplored. No substantial controlled trials of any regimen are currently avail-able. In the United States, there are no approved pharmacological therapies for osteoporosis in men. As a result of this dirth of information, recommendations must come from assumptions based on the current understanding of bone biology and pathophysiology and from the much larger experience in women. Because of gender differences, the preventative and therapeutic strategies developed in women may have unexpected results when used in men. For instance, most therapeutic experience available is in postmenopausal women, a group that has developed osteoporosis through etiological mechanisms probably quite distinct from those affecting most men with the disorder. Morever, osteoporotic postmenopausal women may have bone remodeling dynamics that are distinct from those in similarly aged osteoporotic men and very different from young men with osteoporosis.
